- Applicants must have a diagnosis of developmental disability: Such as downs syndrome, mental retardation, autism, or another neurological condition which requires similar treatment and that originates before the age of 18.
- Must be at least eighteen (18) years of age.
- Provide medical information to include a recent (within one year) physical examination, and current medication list.
- Individual must enter admission with a current photo I.D.; Social security card; certified copy of birth certificate; and when applicable a copy of appointed legal guardianship.
- Copy of a recent (within one year) psychological assessment to include I.Q. test results and reliable and recognized measure of adaptive functioning.
- Here at Santa Fe Place we provide a safe environment for your loved one to live. Individuals must demonstrate a positive stable behavior pattern, free from physical or verbal aggression. Violence or physical aggression will be deemed inappropriateĀ behavior for placement.
- Applicant must be able to ambulate independently and evacuate without physical assistance.
- Applicants with an I.Q. of 59 or above and significant impairments in adaptive functioning will require pre-approval from the State before acceptance.
- Individual must demonstrate willingness to work toward established goals and objectives in order to participate in their active treatment, and to some extent enjoy the company of others in a small group setting.
- Individuals that are accepted for placement will be:
- Place on a waiting list if no units are available.
- Subject to approval for placement from the Oklahoma State Health Department prior to admission.
- Placed in a ninety (90) day trial period.
- If the individual has ever been convicted of any sexual crimes or has a record of assault and battery, the individual will be deemed as unqualified for residency.
